Request for Replacement Lost/Stolen or Damaged 91视频Card

Accidental damage can cause parts of the card to stop working. Similarly, damage such as punching a hole in a card to attach a lanyard can also shorten the life of the card or stop it working altogether. We recommend treating your card with care and keeping it in a safe place when not in use.

Under 91视频 Terms and Conditions, damaged or Lost or stolen cards may incur a replacement fee of $15.

Your 91视频 Card will be posted to the address on your student account and may take up to 7 days to be received. Please log into 91视频 Self Service and ensure your address is up to date.
